YÖK 100/2000 PROGRESS REPORT

For PhD / Proficiency in Arts Scholarship Students

SCIENTIFIC PREPARATORY PROGRAM REGISTRATION FORM

STUDENT ID CARD PRINTING REQUEST FORM

EXTERNAL COURSE ENROLLMENT REQUEST FORM (FROM ANOTHER UNIVERSITY)

Must be completed by the student and submitted to the Department / Department of Arts Chair.

SUSPENSION REQUEST PETITION

Must be completed by the student and submitted to the Graduate School Directorate (either in person to the document registry office or via e-mail to lee@eskisehir.edu.tr).

IMPORTANT NOTE FOR INTERNATIONAL STUDENTS: In case of suspending your registration even for a single semester, your student rights are revoked immediately. Thus your residence permit will no longer be valid and you may not be able to stay in Türkiye.

DEREGISTRATION REQUEST PETITION

Must be completed by the student and submitted to the Graduate School Directorate (either in person to the document registry office or via e-mail to lee@eskisehir.edu.tr).

COURSE TRANSFER FORM

For course transfer and exemption requests (excluding the Scientific Ethics and Research Techniques course), the form must be completed by the student and submitted to the Department / Department of Arts Chair by the end of the course registration week of the semester in which the final enrollment is completed.

PETITION FOR EXEMPTION FROM SCIENTIFIC ETHICS AND RESEARCH TECHNIQUES COURSE

Must be completed by the student and submitted to the Directorate of the Graduate School (either in person to the document registry office or via e-mail to lee@eskisehir.edu.tr).

MAKE-UP EXAM APPLICATION FORM

Must be completed by the student and submitted to the Graduate School Directorate (either in person to the document registry office or via e-mail to lee@eskisehir.edu.tr).

SUBSTITUTE COURSE SELECTION / SUBSTITUTE COURSE PROCEDURE CORRECTION REQUEST FORM

Must be completed by the student and submitted to the Directorate of the Graduate School (either in person to the document registry office or via e-mail to lee@eskisehir.edu.tr).
